Homebound offers various services to the students of the Scottsdale School District:

  1. A student qualifies for Homebound if a medical doctor completes a form with a diagnosis that prevents a student from attending school for a period of at least 3 months.
  2. A student will qualify for Homebound-Intermittent if he/she has an illness that limits attendance at some times, while at other times they are able to attend school. An example would be a student undergoing chemotherapy who is only unable to attend school for the periods surrounding the treatments.
  3. The Scottsdale Unified School District makes Homebound available to pregnant students who have medical complications with their pregnancy or medical complications to the newborn baby.

PURPOSE AND QUALIFICATIONS
The purpose of Homebound Services is to provide education to students of the Scottsdale Unified School District who cannot attend school due to illness, disease, accident or other heath conditions.  Homebound provides quality education with caring teachers to students who meet the requirements for enrollment. All Homebound teachers hold an Arizona teaching certificate.

Each homebound student receives a minimum of four hours of instruction per week.  High school students receive one hour of instruction each week per subject. Middle and elementary students receive five hours of instruction per week. Each teacher schedules directly with the parent.  The goal is to find a time that is best for student, parent, and teacher.  Homework and grades are generated by the Homebound teachers.  Homebound students receive their textbooks from their home school.

Requirements for a student to be admitted into the Homebound Program include a completed application signed by a licensed medical doctor.  By signing the form, the physician is certifying that the student will need to be out of school for at least three school months (sixty school days) relating to the diagnosis.  These days can be consecutive or intermittent, depending on the needs of the student.  The physician must list a diagnosis, telling the last date he/she treated the student (must be within one month of receipt of application), and the date the student could return to the regular classroom. 

Pregnant students may be admitted for less than sixty days if the doctor certifies that there is a risk to the pregnancy or the student’s health. Student may remain on Homebound until her physician states that she is physically able to return to school.  New mothers may receive services if a doctor certifies that their baby is ill and needs their constant care.

IEPs may place students on Homebound or Home Instruction.

Note that a student does not have to miss a certain number of days before going onto Homebound.

Homebound applications are available on the district website: www.susd.org under Student Services, Homebound Health.  Applications may also be mailed or faxed to parents/doctors from the Homebound office, or obtained through the school counselor’s office.

 

POLICIES AND PROCEDURES

  1. An adult over the age of 21 must be present during instruction.
  2. The student should accept the responsibility for completing all work assigned prior to the teacher’s arrival in order to receive credit and grades for assignment/courses taken. 
  3. Student must work with the teacher(s) in a cooperative manner.
  4. Scheduled class times must be adhered to.  Missed classes may be rescheduled, but excessive missed classes may result in suspension of homebound.
  5. At any time a teacher and/or student feels uncomfortable, teacher may leave the premises.
  6. Locations other than the home may be used for instruction if it is more convenient for both teacher and student or if the home is not conducive to a learning environment.  These must be public facilities such as a library.
  7. Any suspicious activities observed by a teacher in a home will be reported to appropriate authorities; e.g., use of alcohol by minors, child abuse, etc.

REASONS FOR TERMINATING CLASSES/SERVICES

  1. Refusal of student to complete assignments on a regular basis.
  2. Failure of student to keep scheduled appointments with teachers with no prior notification.  After two no-shows, the student/parents will be warned; after three, student will be dropped from that class.
  3. Expiration of application and no renewal by doctor.
